We can accommodate up to 72 players on a first-come (meaning PAID) first-served basis.

WAIVER AND RELEASE
The undersigned agrees that I voluntarily enter this golf event at my sole risk, and that the North Phoenix Bar Association, its affiliates, subsidiaries, members, board of directors; their heirs and Tatum Ranch Golf Club, the organizers, sponsors and volunteers of this event (collectively, the “Hosts”) are exempt from all liabilities whatsoever for any damages, injuries and loss, including personal and property loss, that I may sustain arising from or in connection with any cause whatsoever, including, without limitation, negligence. In consideration of the acceptance of my entry to the golf event by the Hosts, I, for myself, my heirs, executors and administrators: (i) waive all rights and claim for damages sustained by me as a result of this event for any cause whatsoever, including, without limitation, negligence; and (ii) release the Hosts from all liabilities associated therewith and indemnify them for same. I warrant that I am physically fit to participate in this event. I hereby recognize that my photograph, electronic image and/or video image may be captured during this event. I give permission to the Hosts to use these images through any media at any point in time. I hereby assign and transfer to the Hosts all rights, including copyright, which I may now or hereafter have on such images. By signing below, I acknowledge having read, understood and agreed to the above waiver, release and indemnity.
